Dear friends and followers, welcome back to my channel!
On the 20th of February 2021, a United Boeing 777-200, registration N772UA performing flight UA-328 from Denver,CO to Honolulu,HI (USA) with 231 passengers and 10 crew, was in the initial climb out of Denver's runway 25 when the right hand engine's (PW4077) inlet separated associated with the failure of the engine. The crew declared Mayday reporting an engine failure. The aircraft stopped the climb at about 13000 feet, the crew requested to return to Denver after running the checklists. ATC offered any runway, they would make it happen. The aircraft returned to Denver for a safe landing on runway 26 about 23 minutes after departure. The aircraft stopped on the runway for a check by emergency services. Emergency services advised of an active fire within the right hand engine and extinguished the fire a few minutes later. The aircraft was subsequently towed off the runway to a remote parking stand, where passengers disembarked and were bussed to the terminal. There were no injuries.
The engine inlet fell into the neighbourhood of Broomfield,CO, located about 16nm west of Denver near 13th and Elmwood Street, the debris also struck through the roof of an adjacent house.
Broomfield police reported that although debris impacted the neighbourhood and damaged a number of homes, there were no injuries on the ground. The debris field expands over a nautical mile.
Ground observers reported hearing the sound of an explosion like bang, smoke and saw the debris falling down. The aircraft continued flying.

I am amazed that abbreviations are permitted on a checklist. Surely in any emergency, it is possible a non-pilot may be executing these instructions? Also, when people are leaning towards panic, the LAST thing your brain wants to do is remember so random acronym or abbreviation. What the hell is A/T? IMHO, very stupid.

A very good video. The crew of United were exceptional. The danger arose and it was dissipated by the book. ATC were fantastic in their ability to minimise transmissions letting the pilots sort the problem. Congratulations to all for the professionalism and safe outcome.

Sorry, the fire extinguishing agents do not affect the internal engine, but only external. Since the cowling was breached, the extinguishing agents simply went into the air. The flames were simply burning excess fuel and oil from the damage. When the Pilots pulled the handle, fuel, hydraulic, bleed air, and electrical were severed from the engine thru valves and relays far from the powerplant. Still a great video for most who fly but have no idea. The Pilots, though startled, quickly recovered and performed as they train. Awesome job by the crew and the aircraft as well!

It failed safely. So that's nice. It could have been far worse. Sure, it should have been better. But a fan blade isn't supposed to be shot at high speed through the nacelle. I still am not sure if the nacelle was supposed to sheer off. Would containing it really have been better? 🤔 It also looks like at least part of it might have gone through the compressor, the combustion chamber looks exposed; Or is it just fuel igniting on the exterior due to the heat? 🧐 I like planes :|

Per the NTSB, the engine was a Pratt and Whitney. It did have a fan blade break off, believed to be due to metal fatigue. I believe all 777s with the Pratt and Whitney engines have been grounded for inspections.

Shut off the fuel once I recognise this catastrophic failure karma and I know the ring was like and then I'm going to fire extinguishers I'm not going to fire extinguishers when I'm no on pumping the fuel into the engine and then if we're hydraulic system on my isolator hydraulic system in oil so it doesn't get to the other engines

Kind of terrifies me to think, but given that this plane was headed to Hawaii, what would have been the likely outcome if this same incident happened at the halfway point between California and Hawaii where they are 2.5 hours from the closest runway? Could they have flown for that long with that shaky engine?

A few things: Some Boeing types refer to engine number , and some like the B777 by position left/right. Therefore there is no normal method. It is specific to aircraft type. Accelerating to 300 knots would not necessarily require and increase in thrust. The most likely thrust mode at the time was climb thrust which is typically less than thrust at take-off. The pilots decided to turn left . This has nothing to do with which engine failed. Asymmetric thrust does not significantly effect aircraft of this type when manoeuvring. Most likely the pre-flight briefing had determined that as a plan or terrain avoidance was the primary reason. When you state that determining if the engine was secure it would show on the EFIS. On the B777 it would show on the EICAS.

Aerospace is like this. We don’t have crashes, we have “mishaps.” Engines don’t catch on fire, they have “rapid oxidation events.” Pilots don’t hit mountains, we have “controlled flight into terrain.” 🤷🏻♂️ Blades don’t break off, they are “liberated.” Be free, little compressor blades, be free!! 😆

They more than likely would have used the QRH Engine severe damage/ separation checklist , unless UAL does things differently than other airlines. An emergency situation like this calls for the Quick Reference Handbook. The fire non-normal checklist is similar, but does not go into the vibration which was quite obviously there.
